Chinese Acupuncture & Moxibustion ; (12): 32-36, 2020.

ABSTRACT

OBJECTIVE@#To compare the analgesic effect of bloodletting acupuncture at -well points along three- meridians of foot combined with routine acupuncture and simple routine acupuncture on migraine.@*METHODS@#A total of 60 patients with migraine were randomized into an observation group and a control group, 30 cases in each one, of which, 4 cases were dropped out in the observation group, 1 case was dropped out in the control group. In the observation group, bloodletting acupuncture at -well points combined with routine acupuncture were applied. The bloodletting acupuncture was applied at corresponding -well points of three- meridians of foot [Lidui (ST 45), Zhiyin (BL 67), Zuqiaoyin (GB 44)] according to pain location. And routine acupuncture was adopted at Sizhukong (TE 23), Shuaigu (GB 8), Taiyang (EX-HN 5), Fengchi (GB 20), Hegu (LI 4), Taichong (LR 3), Zulinqi (GB 41), Yanglingquan (GB 34) and Waiguan (TE 5). In the control group, routine acupuncture was applied, acupoint selection and operation were the same as the observation group. The treatment was given once a day, 30 min a time, 5 days as one course with 2 days interval, and 2 courses were required. Before treatment, immediately after needle withdrawal, 4 h after needle withdrawal and after 2 courses of treatment, the visual analogue scale (VAS) score was compared in the two groups. Before and after treatment, the migraine comprehensive score was observed in the two groups, and the therapeutic effect was evaluated.@*RESULTS@#Immediately after needle withdrawal, 4 h after needle withdrawal and after 2 courses of treatment, the VAS scores in the two groups were decreased (0.05).@*CONCLUSION@#Bloodletting acupuncture at -well points along three- meridians of foot combined with routine acupuncture and simple routine acupuncture have analgesic effect, and the combined therapy is superior to simple routine acupuncture.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate whether periodontal infection is a risk factor for lower respiratory infection patients receiving oral and maxillofacial tumor surgery.</p><p><b>METHODS</b>Patients undergoing oral and maxillofacial surgery for tumors with concurrent periodontal disease between January, 2012 and December, 2016 were randomized into periodontal intervention group and control group (treated with gargle solution containing chlorhexidine gluconate). The one-time periodontal intervention was completed within 24 h in the intervention group and two-week mouthwash was prescribed in the control group before oral and maxillofacial tumor surgery. Five periodontal indexes were examined at baseline and at 6 weeks after the treatment. The clinical symptoms and incidence of lower respiratory infections were compared between the two groups at 6 weeks.</p><p><b>RESULTS</b>The PLI, BOP, PPD, and CAL were significantly lower and GR was higher in the intervention group than in the control group (P<0.01). The periodontal status was significantly improved in the intervention group. The incidence of lower respiratory infection was significantly lower in the intervention group than in the control group (2.22% vs 7.11%, P<0.01). The incidences of cough and expectoration in the intervention group were significantly lower than those in control group (P<0.01).</p><p><b>CONCLUSIONS</b>Periodontal infection is one of the risk factors for lower respiratory infection after oral and maxillofacial tumor surgery. The periodontal status can be effectively controlled and improved by periodontal intervention. Compared with mouthwash, periodontal intervention can significantly reduce the incidences of cough and expectoration and lower the incidence of lower respiratory infections.</p>

ABSTRACT

Objective To explore the association between one common variant in ABCB11-1331T > C (V444A) and neonatal cholestasis.Methods One hundred and ninety-two children with neonatal cholestasis were enrolled as case group,and 196 healthy children were selected as healthy control group.The SNP site of V444A was tested by fluorescent quantitative PCR.Fisher's exact test was performed to detect the differences in allele and genotype distribution between the 2 groups.Wilcoxon rank-sum test was used to test the differences of total bilirubin,total bile acid,γ-glutamyl transpeptidase levels among the patients with different genotypes.Results TT,TC and CC genotypic distribution of V444A were not significantly different between patients and controls (P =0.530).The T allele in the case group accounted for 29.9%,in the healthy control group accounted for 26.3%,there was no significant difference between the 2 groups(OR =1.12,P =0.264).Total bilirubin,total bile acid,γ-glutamyl transpeptidase levels in patients with different genotypes of V444A were also not statistically different (all P > 0.05).Conclusion Only V444A variant may have no impacts on neonatal cholestasis.

ABSTRACT

A simple, fast and sensitive analytical method for the simultaneous separation and detection of 18alpha-glycyrrhizinic acid, 18beta-glycyrrhizinic acid, related substance A and related substance B by RP-HPLC and drug quality standard was established. The structures of principal component isomer and related substances of raw material drug of ammonium glycyrrhizinate have been confirmed. Reference European Pharmacopoeia EP7.0 version, British Pharmacopoeia 2012 version, National Drug Standards of China (WS 1-XG-2002), domestic and international interrelated literature were referred to select the composition of mobile phase. The experimental parameters including salt concentration, pH, addition quantities of organic solvent, column temperature and flow rate were optimized. Finally, the assay was conducted on a Durashell-C18 column (250 mm x 4.6 mm, 5 microm) with 0.01 mol x mL(-1) ammonium perchlorate (add ammonia to adjust the pH value to 8.2) -methanol (48 : 52) as mobile phase at the flow rate of 0.8 mL x min(-1), and the detection wavelength was set at 254 nm. The column temperature was 50 degrees C and the injection volume was 10 microL. The MS, NMR, UV and RP-HPLC were used to confirm the structures of principal component isomer and related substances of raw material drug of ammonium glycyrrhizinate. Under the optimized separation conditions, the calibration curves of 18 alpha-glycyrrhizinic acid, 18beta-glycyrrhizinic acid, related substance A and related substance B showed good linearity within the concentration of 0.50-100 microg x mL(-1) (r = 0.999 9). The detection limits for 18alpha-glycyrrhizinic acid, 18beta-glycyrrhizinic acid, related substance A and related substance B were 0.15, 0.10, 0.10, 0.15 microg x mL(-1) respectively. The method is sensitive, reproducible and the results are accurate and reliable. It can be used for chiral resolution of 18alpha-glycyrrhizinic acid, 18Pbeta-glycyrrhizinic acid, and detection content of principal component and related substances of raw material drug of ammonium glycyrrhizinate. It is concluded that the separation of principal component isomer of raw material drug of ammonium glycyrrhizinate and the validity of the substance's structure assignments of retention time being 1.2 in the European pharmacopoeia EP7.0 version, British pharmacopoeia 2012 version remains open to question. It may be of practical value for the quality control of raw material drug, preparation, and Chinese herbal medicine of ammonium glycyrrhizinate.

ABSTRACT

<p><b>OBJECTIVE</b>To explore whether SLC25A13 gene mutation exists and what is its mutation spectrum in mainland Chinese infants with intrahepatic cholestasis and abnormal blood amino acids.</p><p><b>METHODS</b>Blood amino acids were analyzed by mass chromatographic analysis in infants referred to Fudan University Children's Hospital from June 2003 to June 2007 for investigations of intrahepatic cholestasis of unknown origin. SLC25A13 gene mutations were studied in 14 children whose serum levels of citrulline and/or methionine were at least two times above the upper normal range. In patients in whom only one mutation was detected, all other exons and their neighboring sequences were then analyzed.</p><p><b>RESULTS</b>Eight patients with SLC25A13 gene mutations, including 2 with compound heterozygous mutation 851del4/1638ins23, one with homozygous mutation 851del4/851del4, one with compound heterozygous mutation 851del4/R184X, one with homozygous mutation IVS6+1G more than A/IVS6+1G more than A, and 3 with heterozygous mutation 851del4 were found.</p><p><b>CONCLUSIONS</b>SLC25A13 gene mutations exist in Chinese infants with intrahepatic cholestasis and abnormal blood amino acids. Their mutation spectrum is different from that in Japan. 851del4 is the most common mutation in our study. IVS6+1G more than A is a mutation that has not been reported before.</p>

ABSTRACT

ABCB11 gene encodes bile salt export pump (BSEP).It is almost exclusively expressed in the canalicular microvilli of liver.It is the principal conveyor of bile acids from hepatocyte cytoplasm into bile canaliculus.It is clearly that BSEP defects can induce progressive familial intrahepatic cholestasis type 2 and benign recurrent intrahepatic cholestasis type 2.ABCB11 gene variation are also responsible for intrahepatic cholestasis of pregnancy,drug-induced cholestasis,primary sclerosis cholangitis and primary bile cirrhosis.This paper reviewed the association of ABCB11 gene variation and these diseases.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the long-term efficiency between preoperative radiotherapy or radiochemotherapy followed by lower-anterior resection and abdominoperineal resection (APR) for lower and locally advanced rectal cancer.</p><p><b>METHODS</b>From January 1983 to December 2000, 157 consecutive patients suffering from lower rectal cancer were enrolled in the study, which included 69 cases of clinical stage II and 88 cases of stage III respectively. All patients were divided in to three groups. Patients in group A (n=52) received preoperative radiotherapy with a total dose of 35-45 Gy within 4-5 weeks plus preoperative chemotherapy with 5-fluorouracil (5- FU) followed by lower-anterior resection; patients in group B (n=51) received radiotherapy followed by lower-anterior resection; patients in group C (n=54) received APR only. Clinical data of all patients were reviewed retrospectively.</p><p><b>RESULTS</b>The follow-up rate was 91.7%. The 5- year survival rate was higher in group A (71.1%) than those in group B (47.1%) and group C (42.6%)(P< 0.05). The tumor- free survival rate was higher in group A (61.5%) than those in group B (37.3%) and group C (35.2%)(P< 0.05). The local recurrence rate was 13.5%, 15.7% and 11.1% in group A, B and C respectively, there was no significant difference in recurrence rate among three groups (P> 0.05). The distant metastasis rate was lower in group A (23.1%) than those in group B (49.0%) and group C (46.3%)(P< 0.05), but there was no significant difference in distant metastasis rate between group B and group C.</p><p><b>CONCLUSIONS</b>The combined preoperative radiochemotherapy followed by lower-anterior resection can improve the 5-year survival rate and tumor-free survival rate, and decrease distal metastasis rate.</p>
